REFERRALS
In the state of Washington a referral, or prescription, is not required to receive physical therapy services, however, it may be required by your health insurance company. If you would like to self-refer to physical therapy we recommend you contact your health insurance company to see if a referral is necessary.

Referrals from physicians and other healthcare providers, case managers, vocational counselors, patients, attorneys, and insurance professionals are the cornerstone of our practice. If you know of someone who need physical therapy services, you may complete a referral by downloading a PDF version of one of our referral forms.
